OSHA Form 200 Replacement is a free FileMaker Pro 3.0 or greater
database that will generate the necessary forms for small businesses
to comply with OSHA requirements. It is quick and relatively easy to
use if you follow the directions. I hope you find it useful. I'd like

OSHA Form 200 Replacement for FileMaker Pro

1. Choose the "Company Information" layout and enter your company
information. You will be prompted for this when you first open the
database. This info is stored in global fields, so if you have two or
more stores/locations/businesses, you will need to create separate
copies of the database for each one.

Just remember a few things:
i. Each injury or illness is a separate record
ii. You should always begin creating a new record at the "Enter
Incidents" page
iii. You should do everything in browse mode, unless you have to
fiddle with the print settings (you shouldn't need to) of the "File"
and "Post" layouts
iv. Each record requires the complete company info: name, address,
etc. OSHA Form 200 Replacement will automatically enter the company
information into the correct fields for the second and subsequent
records. Just use the tab key to go through the fields and don't
enter any info on the "Company Information" layout, then proceed to
"Incidents."

2. Choose "Enter/Edit Incidents" and enter each reportable incident's
data as a new record using the "Create New Incident" button or the new
record command in the mode menu.
From "Incidents," you will go to "Injuries" or "Illnesses." The "Go
To Next Step" button will take you to the correct place. No record
should have both injury and illness information entered. If the same
employee gets sick and gets injured, those are two distinct incidents.

3. Click the "Print Reports" button. (You should not print from the
File menu, the button runs an error check, and the appropriate
layouts are invisible.)
There is no error checking for dates of incidents! You will have to
ensure that the dates you enter are correct, and that the only
records present in the file are pertinent to the year you are
printing reports for. This was intentional, because an incident can
span two (or more) years and might require entries on separate years
reports.

When the "Print Reports" button is clicked, FileMaker will find all
the records, sort by date, and print "File" page(s) then "Post"
page(s). The Post page must be posted from February 1st through March
1st of the year following the incidents. If there were no reportable
incidents, click the "Print 'Zero Incidents' Reports" button. This
will create one record with N/A or zeroes as appropriate. After March
1st, retain the "Post" page with your records and make a clone (in the
Save As... dialog box under the File menu) of OSHA Form 200
Replacement to begin the following year's file.

The forms are set up to print correctly on both an Apple Personal
LaserWriter NTR and an HP Deskwriter 560C. They should print
correctly on virtually any printer. At least, I hope they do. 

4. Detailed instructions on the maintenance and retention of OSHA
Form 200 are contained in OSHA Form 200 Replacement on the
"Instructions" layout. More information and sample Form 200s are
available from your state's Labor or Insurance Departments.

as long as all its ancillary documents are included and it remains in
